Description:

To be eligible for the retake in Minnesota, you must have taken the full test in Minnesota. If taken in Wisconsin, you will either need to register for the full test in MN or take a retake in WI.

Bring to Class:

NOTE: Testing times will be staggered between 2 PM - 8 PM. You will be notified of your test time 2-3 days prior to testing. Your time may be adjusted, depending on cancellations or delays. Please arrive 30 minutes prior to your scheduled testing time to check in with the testing coordinator in Room C268. If you fail to arrive on time, we may move onto the next person and you will need to reschedule and repay. If you are unable to make your scheduled test time, please contact Marilyn Slattengren at 218-348-4542.

Requirements

1. Identification: You must bring a UNITED STATES (US) GOVERNMENT ISSUED, SIGNED, UNEXPIRED, PHOTO BEARING ID. ( *See exception below.) Only original IDs are accepted. No photocopies, faxes or images are allowed. Examples of the forms of US government issued, photo ID’s that are acceptable are:

State or other United States Government Issued Driver’s License
State Identification Card (that meets all identification criteria)
US Passport (Foreign Passports and Passport Cards are not acceptable) •Exception: A Foreign Passport that contains a US VISA is acceptable ? Alien
Registration Card (that meets all identification criteria; may contain a fingerprint in place of signature)
Tribal Identification Card (that meets all identification criteria)
Work Authorization Card (that meets all identification criteria)
Military Identification (that meets all identification criteria)
Identification Criteria = US Government issued, signed (or fingerprint – Alien Registration Card), unexpired, photo bearing form of identification

*EXCEPTION: If you do not have a current, official US government issued identification form, then you must bring two (2) forms of CURRENT (not expired), OFFICIAL, SIGNATURE BEARING identification with matching names on both IDs. One of the two forms must be PHOTO-BEARING (photo copies will not be accepted). Examples of proper identification, with matching names, include CURRENT (non-expired):

Signed Voter Registration Card
Signed Credit/Debit (Bank) Card
Signed Social Security Card

2. Please arrive at the testing room at least 15 minutes prior to the start time. We reserve the right to refuse test takers who arrive past the start time and refunds will not be issued.

4. Wear a mask. Masks are required while testing at LSC.

What to Wear:

Scrubs are REQUIRED. Dress as if you were already on the job as a competent CNA. Wear a neat short-sleeved scrub uniform and clinical shoes with non-skid soles. Don't forget a watch with a second hand. If your hair is long, tie it back. Leave your jewelry at home. Your nails should be short. Wearing a mask is required.

Parking/Entrance:

Parking is free and available to you in the East Parking Lot. Please do not park in reserved parking. The closest entrance to your room is the Main Entrance. Go up the first set of stairs on your right, once up the stairs, the testing room will be straight down the hall on your left.

Cancellation/Refund Policy:

Full refunds will be issued if session is canceled due to low enrollments or inclement weather. Refunds will be issued if the test date is dropped or date changes are made 3 days prior to test date. To drop or reschedule, please call please call 218-733-7680 or continuingeducation@lsc.edu.

Inclement Weather:

If class is cancelled for the campus, so are NATO exams. Such announcements will be posted to the Lake Superior College’s website (www.lsc.edu). Cancellations will also be announced on the following radio and commercial television stations:
KDAL-AM, 610- AM; KDAL-FM, 95.7-FM; KKBC-FM, 105-FM; KLDJ-FM 101.7-FM; KTCO-FM, 98.9-FM; KRBR-FM, 102.5-FM; WDSMAM, 710-AM; WEBC-AM, 560-AM.

Accommodations:
